Health & Social Care Jobs have been engaged to recruit a Field/Care Manager for a Homecare company based in Chiswick, West London. Car Driver is Essential (Access to a company pool car)

£28-£32K which includes a £3900 yearly on call allowance within the salary

 

Location

From their office they deliver their homecare services across the Ealing, Acton, and Chiswick areas of West London. This job vacancy is commutable from Fulham, Putney, Twickenham, Richmond, Hounslow, Brentford, Acton, Ealing, Wembley, and surrounding areas.  With fantastic transport links, this position is commutable by car, or the London Underground (Chiswick park or Gunnersbury via the District Line) or Turnham Green or Gunnersbury for the over Ground Station).   There is no onsite parking available (hence the use of the pool car)

Your Role

As their Field/Care Manager, you will oversee approximately 15 clients, meaning you can really focus on building a relationship and giving your clients the quality time, they need.  You will be responsible for:

  • Carrying out client reviews.
  • Working in the community carrying out spot checks.
  • Quality Audits
  • Liaise with MDT Teams to discuss changes.
  • Care Coordinator when on call (1 Weekend in 4 and 1 evening per week)

Qualities required

  • Previous Experience of working with Older People / Dementia or Palliative Care.
  • Ideally previous experience of working within the community / Homecare
  • Someone who may become a Dementia Champion / medication champion
  • A minimum qualification of Level 3 in Health & Social Care
