We are looking for a Band 5 Complex Care Nurse Manager is to join a well-established team who cover the South of Greater London. A car driver is essential for this role. This job vacancy is ideal for someone who has experience within the NHS as an ICU, Critical Care (CCU) with experience of complex clinical care needs.
£42-45K per annum, Monday – Friday 9am – 5pm, Plus Pension Scheme

The Job Role
You would be working in their Field Based, Complex Care Team close to your home in the South London Area. As a Nurse Case Manager your role would be providing face to face case management and support to their clients and families. (Supporting no more than 15 Clients). Initially assessing your clients prior to leaving hospital, then visiting them at least every four weeks, within their own homes to assess and review the care plan in place. You will also create very person-centred care plan for each individual to maximise the quality of the care you and your teams provide. You would work closely with a core team of carers training and supporting them with clinical skills and activities. As a Nurse Case Manager your role is pivotal within the community and you will be working very closely with outside agencies, multi-disciplinary teams, commissioners, and CCG’s to ensure bespoke packages of care are delivered to a high standard.
Key requirements for the role are:
- A minimum of Band 5 / Minimum of 2 years clinical experience
- Registered Nurse (RN) with an active NMC Pin Number
- Tracheostomy and Ventilator experience is essential.
- Excellent written and verbal communications skills as well as attention to detail.
- An excellent supportive manager with demonstratable experience of supervising a team.
- You will be confident, with a passion for high quality care and a can-do attitude that you cascade to others.
