Care Coordinator

£23K - £24.5K per annum DOE + bonus

Full-time, permanent job vacancy, Care Coordinator, Norwich, Norfolk.

We have an exciting opportunity for a talented Care Coordinator to join a fantastic, award-winning homecare provider in the Norfolk area.

The Provider

We are recruiting on behalf of a homecare/domiciliary care provider founded just over 6 years ago. They have won numerous awards over those years and are incredibly proud of the high-quality they provide each and every day. Rated Good by the CQC, they aim to continue going from strength to strength. As a provider, they know what an amazing job both their field-based staff and office team do, so they offer them amazing rewards because they deserve it. This year they gave away a car and 35 weekend breaks – amazing. The culture here is not only fantastic, but also open and supportive. Their great staff retention is testament to this. Most of the office team started in other roles and progressed into their current roles. There is low staff turnover, and the majority of the team are their long-term.

Key Information

  • £23K - £24.5K per annum DOE + bonus
  • Full-time, Monday – Friday including on-call 1 in 4.
  • 1500 care hours delivered per week.
  • 66 care staff working each week.
  • Level 3 Health & Social Care is desirable.
  • Minimum 12-months experience as a Care Coordinator.
  • Experience in homecare/domiciliary care setting.
  • Being a car driver is essential.

Your Role

As their Care Coordinator, your role will be key to the successful running of the service, ensuring all calls are filled. The role will include:

  • Identify suitable matches for clients.
  • Using PASS system to schedule weekly and monthly rotas.
  • Accepting, allocating and processing referrals for new work.
  • Talking to the customers on a regular basis about their care.
  • Identify recruitment current and future needs.
  • Take part in the on-call rota on a rota basis (1 in 4).

    Could you help Mavis?

    Mavis sadly lost her husband a few years ago, she has managed at home with the help from her close relatives and family friends. She now needs some extra support and assistance to be able to stay at home in the place she loves. You could help her with getting out of bed making sure she does not trip or fall, assisting her into the bathroom and prompting her with her morning wash and brushing her teeth. Once safely downstairs you will assist with making Mavis her breakfast which is normally porridge and a few blueberries. Mavis is great with taking her medication but just needs little reminders of the day and time. Mavis loves knowing someone is coming throughout the day not only to support her but to just have a chat with her and keep her company.
