Job description

Are you an experienced Adult Mental Health Support Worker, living in the Bognor Regis area of West Sussex?  This job vacancy is also commutable from Chichester, Worthing, or the Havant area of Hampshire.

Key Information

  • £21’400 per annum.
  • Contributory Pension Scheme.
  • Full Time, Permanent, Job Vacancy.
  • 37.5 Hours per week.
  • Previous experience working with Adults with Learning Disabilities/Mental Health.
  • Full UK Driver’s license and access to your own car.
  • All applicants must have their 2nd Covid vaccine / or arranged.

Your role

  • Establishing and maintaining a regular presence at primary care practices.
  • Working in partnership with key practice staff, clinicians, and community-based services.
  • Providing timely support to people identified as requiring support.
  • Using established process to assess the requirements of people in need.
  • Supporting people to identify their needs and goals.
  • Developing support and recovery plans.
  • Helping people access community-based resources and services.
  • Maintaining accurate and up to date records.
  • Ensuring compliance with multi-agency policies around safeguarding adults and children.
  • Follow all health and safety regulations.
  • Undertaking training identified as necessary for the role.
